In AI systems, a hallucination is typically defined as a generated output that is fluent and plausible but not grounded in verifiable data or reality.
In humans, hallucination can refer to perceptual distortions, but in everyday thinking, it more often shows up as false memories, overconfident inferences, or stories that feel true without being fully accurate.
This distinction matters because, even though the mechanisms differ, both AI and human hallucinations involve prediction filling gaps under uncertainty.
These human phenomena also come from different cognitive systems: false memories come from reconstructive memory, perceptual distortions from sensory prediction errors, and overconfident conclusions from reasoning biases and mental shortcuts. They can be grouped conceptually, but they rely on different mechanisms.
The idea that the brain constructs and predicts rather than simply records reality closely mirrors a central theory in cognitive science known as predictive processing: the brain is not a passive recorder of reality. Only later did the term narrow to mean false perception, such as seeing things that aren’t there. In its original sense, hallucination was not just about perception, but about cognition more generally: the mind filling gaps, losing its way, and constructing something that feels real but isn’t fully grounded.
Everyone thinks AI hallucinations are a bug, not a feature. Many people, from tech leaders to everyday users, see AI hallucinations as a fatal flaw, proof that AI can't be trusted.
But here’s the uncomfortable truth: hallucinations aren’t just an AI problem. They’re a human problem.
Your brain hallucinates constantly. It interprets, fills in, and reconstructs. It misremembers events. It fills in gaps. It constructs entirely false memories that you confidently swear are real. Eyewitness testimony is often less stable and less reliable than people assume.
I’ve had this happen myself: remembering something with absolute certainty, only to realize later it never happened that way.
Yet we do not dismiss human intelligence because of this. Instead, we check facts, correct mistakes, and work with our mental glitches.
More precisely, memory is reconstructive rather than reproductive. People don’t retrieve memories like files from a hard drive. They rebuild them from fragments, expectations, emotion, and context. That is why confidence and accuracy in human memory are often far less correlated than we assume.
Sure, we need to be careful here: human and AI hallucinations are not the same phenomenon. Human errors come from embodied experience, perception, emotion, and reconstructive memory, while AI hallucinations come from statistical pattern completion without grounding in lived reality. They are different in form, but they arise for the same reason: both systems operate under uncertainty and fill in missing information.
That said, “uncertainty” plays out very differently in each case. Human cognition integrates perception, action, feedback, and social correction in a continuous loop shaped by embodiment and experience. AI systems, by contrast, generate outputs without direct experiential anchoring or real-time grounding in the world. Treating them as identical would obscure more than it explains.
Another difference is embodiment: Human cognition is shaped by a body moving through the world. Perception, action, and feedback stay tightly linked. AI systems lack that embodied grounding, which limits how well they can anchor predictions in real-world constraints. This is one reason their “hallucinations” can drift further from reality than typical human errors.
A major difference lies in how these systems get corrected. Humans operate in a closed loop. We test predictions constantly - through perception, through other people, through experience. AI systems, by contrast, often work in a more open-loop way at inference time, generating outputs without direct grounding in real-time feedback. This makes their errors less self-correcting and more persistent.
There is an even deeper philosophical difference: human thoughts have intentionality. They are about something in the world. AI outputs, by contrast, do not refer to the world in this way. They are patterns that we interpret as if they were about something. When a human is wrong, they hold a false belief. When an AI is wrong, it does not “believe” anything at all. It produces an ungrounded output.
This distinction aligns with long-standing philosophical debates. Thinkers like John Searle have argued that computation alone cannot produce genuine understanding, while others, like Daniel Dennett, take a more functional view. This essay leans toward the position that current AI systems lack intrinsic intentionality, even if they can simulate it convincingly.
And there is another key difference: humans are embedded in social and experiential contexts that allow them to recognize and correct their own errors.
Current AI systems do not exhibit self-awareness and cannot reliably distinguish between useful and harmful outputs, or between a useful hallucination and a dangerous one. That makes risk management crucial.
A strong objection is that AI hallucinations can be more dangerous in certain contexts because they scale and can produce confident errors rapidly. A human might misremember a detail. An AI system can generate thousands of confident falsehoods in seconds. This is not just a difference in degree. It may be a difference in kind. That is exactly why treating hallucinations as “just like humans“ would be a mistake. The real insight is not equivalence. Both require correction, but AI needs far stricter control.
Some philosophers would go even further and argue that calling AI outputs “hallucinations” is misleading altogether. The term suggests a mind that misperceives reality. But AI does not perceive reality in the first place.
From this perspective, hallucination is not an error within cognition - it is a limitation of a system that lacks cognition entirely.
So while both AI and humans hallucinate, there is still a crucial difference: Humans can experience doubt and sometimes correct their errors, though psychology shows we are often far less reliable at this than we assume. We are vulnerable to confirmation bias, overconfidence, motivated reasoning, and hindsight distortion. What makes human correction possible is not flawless self-awareness, but a combination of feedback, contradiction, social accountability, and repeated reality checks.
AI lacks this metacognition. It will state an incorrect fact with the same confidence as a correct one. This is why external safeguards and verification mechanisms are essential.
This points to a central issue: confidence calibration. Humans are often wrong, but their confidence varies. Hesitation, doubt, and uncertainty are signals other people can read. AI systems, however, tend to produce outputs with uniform fluency, which can mimic high confidence even when the content is incorrect. The danger is not just error, but misleading certainty.
AI hallucinations can be highly persuasive. People are more likely to trust information when it is fluent, coherent, repeated, and delivered in an authoritative tone. In other words, AI does not merely generate errors. It generates them in a form people are psychologically primed to accept. If you’ve ever read something that felt right and only later realized it wasn’t, you’ve seen this effect in action.
This connects to a broader psychological principle: cognitive ease. When something is easy to read, sounds polished, and fits our expectations, we are more likely to accept it as true. That makes fluent AI output especially risky, because polished form can hide weak or fabricated content.
And finally, we should not collapse different phenomena into one category. Not every error is a hallucination. A false memory, a probabilistic inference, and a creative leap are related, but not identical. What they share is not their nature, but their structure: each involves constructing an output without complete information.

When Hallucination Is a Strength: The Creative Side of Intelligence
Psychologically speaking, we should be careful with the word “hallucination” here. Creative imagination, pattern extrapolation, false memory, and perceptual distortion are not identical processes. Still, they share an important feature: the mind does not merely mirror reality. It actively generates possibilities, interpretations, and completions.
Not all hallucinations are equal. We can roughly distinguish three types:
Creative hallucinations: generating novel ideas, metaphors, or hypotheses
Benign hallucinations: small inaccuracies that don’t meaningfully impact outcomes
Harmful hallucinations: false outputs in high-stakes domains like medicine, law, or finance
Of course, these examples are not hallucinations in the strict sense. They involve imagination, hypothesis generation, and disciplined iteration. The point is not that all innovation is error, but that the ability to generate possibilities beyond available data, sometimes inaccurately, is tightly linked to creative progress.
The challenge is not to eliminate hallucinations, but to know which type we are dealing with.
In practice, managing hallucinations means combining multiple layers: grounding outputs in retrieved or verifiable data, applying domain-specific constraints, incorporating human-in-the-loop review where stakes are high, and designing systems that communicate uncertainty more transparently. The challenge is not only technical, but also organizational and ethical.
Some hallucinations are useful leaps. Others are misfires.
A child learning language does not say “I goed to the store“ because the child is faulty. The child is extrapolating patterns.
A chess grandmaster sometimes “hallucinates” moves based on intuition rather than calculated precision, and sometimes that wins the game.
Writers hallucinate new metaphors, entrepreneurs hallucinate markets that don’t exist yet, and investors hallucinate company growth that hasn’t happened.
Of course, these examples are not hallucinations in the strict sense. They involve imagination, hypothesis generation, and iterative testing rather than outright error. The point is not that all innovation is built on falsehood, but that the ability to generate possibilities beyond available data, sometimes inaccurately, is closely tied to creative progress.
What is happening in LLMs is similar. Hallucination is a byproduct of prediction under uncertainty - sometimes it lands somewhere brilliant, sometimes it misfires. But unlike a human, an AI doesn’t know when it has made a brilliant leap versus a critical error. Again, that’s why we need guardrails, validation tools, and human oversight to tell useful hallucinations from harmful ones.
Rethinking Facts: Why Certainty Is an Illusion
We often think facts are either true or false, but reality is messier.
Verified and certain: Earth orbits the sun.
Probable but evolving: Economic forecasts.
Socially agreed upon: Market value of Bitcoin.
Indeterminate: The best strategy for an AI-driven business in 2035.
LLMs do not “lie” any more than humans do. They predict probable answers based on patterns in data.
Still, from a user’s perspective, the distinction between “lying” and “generating an incorrect output” may not matter. What matters is the outcome. Systems that produce convincing falsehoods must be treated with the same seriousness as systems that intentionally mislead.
Humans can reason and learn from feedback, which helps them refine their understanding over time. AI, by contrast, does not continuously self-correct in real time.
Humans also do not evaluate truth in a neutral way. Psychological research shows that people often accept information not simply because it is accurate, but because it is comforting, identity-consistent, or useful to believe. That means the AI hallucination problem is never just about model quality. It is also about the minds receiving the output.
AI systems can improve through training, fine-tuning, and external tools, but those corrections usually happen outside the moment of generation, not within it. This creates a gap between producing an answer and evaluating its correctness.
The real tension shows up elsewhere: we expect LLMs to operate with absolute certainty in a world where even humans don’t. This aligns with widely discussed characteristics of modern AI systems. LLMs are generally understood to generate outputs based on probability distributions learned during training, rather than retrieving facts like a traditional database. In that sense, they are closer to predictive systems than knowledge systems, which makes occasional fabrication an expected outcome, not an anomaly.
More specifically, LLMs generate text by predicting the next token in a sequence based on learned statistical patterns. They are not checking facts against a world model. They are extending patterns in the ways their training data makes most probable. This is why outputs can be both highly coherent and completely wrong at the same time.
This raises a deeper epistemic tension: are we optimizing AI for truth or for usefulness? Humans constantly navigate this trade-off: we rely on heuristics, approximations, and sometimes even illusions when they are good enough. AI systems inherit this tension, but without the ability to recognize when “useful enough” becomes dangerously wrong.

Harnessing AI Hallucinations: Turning Misfires into Innovation

This shift in thinking separates those who will struggle with AI from those who will thrive with it.
Rather than asking, “How can we stop AI from hallucinating?” ask:
“How do we harness AI’s creative leaps?”
“How do we build systems that distinguish productive from misleading hallucinations?”
“How can we complement AI’s pattern recognition with human judgment?”
People who get this right will not just keep up with AI. They will help shape its future.
To use AI effectively, we must distinguish between useful and harmful hallucinations. The challenge is not merely technological but also ethical: how do we ensure that AI-generated falsehoods don’t mislead people in critical domains like healthcare, law, and journalism?
From a psychological standpoint, the greatest risk may not be AI hallucination alone, but human-AI interaction. A flawed answer becomes consequential when a person defers to it, repeats it, or acts on it without sufficient scrutiny. The real danger emerges in the loop between machine output and human judgment.
The key challenge is knowing where AI hallucinations are acceptable and where they are dangerous. In creative fields like writing, design, and brainstorming, AI’s generative leaps can be an asset. But in high-stakes areas like medicine, finance, and law, unchecked hallucinations can cause harm. To use AI responsibly, we need both better technology and strong ethical guidelines.
Unlike human error, AI hallucinations raise a unique problem: There is no clear epistemic agent behind them. If an AI produces a harmful falsehood, who is responsible? The user? The developer? The system? This shifts hallucination from a cognitive issue to a moral and institutional issue.
Philosophically, this forces a shift: from asking whether AI outputs are true in isolation to asking how truth is established, verified, and trusted in systems where generation and evaluation are decoupled.

Rethink, Reframe, Reshape: The Future of AI Is in Your Hands

But let’s be clear: AI does not think the way we do. Its hallucinations are mathematical extrapolations, not insights rooted in experience.
The key is to use AI’s strengths while acknowledging its blind spots.
A crucial clarification: pointing out that humans also hallucinate is not meant to downplay the risks of AI. The scale, speed, and perceived authority of AI outputs introduce risks that are qualitatively different from typical human error.
I am not saying we should accept AI errors. I am saying we should understand them in context. AI is a tool, and like any tool, its effectiveness depends on how we use it. Instead of demanding absolute perfection, we should focus on designing systems that maximize AI’s strengths while mitigating its weaknesses.
That is how we make AI not just useful, but trustworthy. If we normalize hallucination as part of intelligence, the real question becomes: When is uncertainty acceptable, and when does it become negligence?
This is not just about improving models. It is about defining standards of truth, accountability, and risk in a world where machines generate knowledge-like outputs without understanding.
In that sense, hallucination is not just a technical flaw. It is a mirror held up to our own epistemic limits, and a test of how seriously we take them.
It is also a mirror held up to our psychological vulnerabilities. We want coherent answers. We trust confidence. We prefer clarity over ambiguity. AI hallucinations exploit precisely the shortcuts that make human thinking efficient, but also fragile.
From a cognitive perspective, intelligence is not about avoiding error. It is about minimizing prediction error over time.
